Nursing Home Costs in Baldwin County, Alabama (2026)

At Baldwin County prices as of 2026 — roughly $7,000 to $8,500 a month for a semi-private skilled nursing room — $200,000 of savings buys about 24 months of gross care, and closer to 34 months once you subtract a typical Social Security check from the burn rate. The same $200,000 in New Jersey or Massachusetts buys about 13 months. Alabama is one of the least expensive states in the country for nursing facility care, and for a family here that is a genuine, quantifiable advantage.

It comes with a constraint that a cost table will not show you. Alabama restricts the construction of new nursing facility beds through its certificate of need process, while Baldwin County has been the fastest-growing county in the state — roughly 182,000 residents in 2010 to about 231,000 in 2020, with continued growth since, much of it retirees moving to Fairhope, Daphne and the Gulf beaches. A long runway does not help if there is no bed to spend it on.

This page does one thing: it builds the runway number honestly, and then says what each range of that number means you should be doing right now. Pine Lake Life Solutions provides education and a free policy review only, and nothing here is legal, tax, or Medicaid-eligibility advice.

The Only Number That Matters: Months, Not Dollars

Families arrive at this problem holding a dollar figure — “Dad has about $180,000” — and dollar figures are almost useless for decision-making. Convert to months and the decisions become obvious.

The formula has three inputs. What a month of the right level of care costs locally. The applicant’s monthly income, which reduces the amount actually drawn from savings. And what genuinely counts as available savings, which is smaller than most people’s mental total. Divide the countable savings by the monthly shortfall and you have months.

Do it in writing, and put the number somewhere the whole family can see. A runway of 40 months and a runway of 9 months call for completely different behavior — one is a planning problem with time to solve it properly, the other is a triage problem where the priority is getting an application filed correctly the first time. Most family conflict about money in this situation comes from different people carrying different unspoken estimates of the same number.

Step One: What a Month Actually Costs in Baldwin County

As of 2026 in Baldwin County, expect these ranges. Skilled nursing, semi-private: roughly $7,000 to $8,500 per month. Skilled nursing, private room: roughly $8,000 to $9,500. Assisted living: roughly $3,800 to $5,500 countywide, with Fairhope and the Eastern Shore running higher at roughly $4,500 to $6,500. Memory care: roughly $5,000 to $6,800. These are ranges derived from cost-of-care survey data and prevailing regional pricing, not quotes.

Two adjustments before you use any of them. First, use the rate for the level of care the person actually needs, not the cheapest rung. A parent who needs two-person transfers is not an assisted living resident, and budgeting at the assisted living rate produces a runway estimate that is wrong by half. Second, add the extras. Level-of-care surcharges, incontinence supplies, therapy cost sharing after a Medicare skilled stay ends, pharmacy, and ancillaries commonly add $300 to $1,200 a month on top of a quoted base rate. Ask each facility for the base private-pay daily rate, the level-of-care surcharge grid, and a list of what is billed separately — in writing.

Also ask what last year’s rate increase was. Increases in the 4% to 8% range have been common through the mid-2020s. A three-year projection built on a flat rate is wrong before you begin; assume the monthly cost rises each year.

Step Two: Subtract Income — the Burn Rate, Not the Bill

This is the step families skip, and skipping it makes the runway look roughly 30% to 40% shorter than it is. The bill is not the burn rate. Monthly income offsets the bill, and savings only cover the difference.

Add up everything that arrives monthly: Social Security, any pension, an annuity payment, rental income, and — importantly in Baldwin County — VA Aid and Attendance if the person is a wartime veteran or the surviving spouse of one. Baldwin County has a substantial veteran population, and Aid and Attendance is routinely left unclaimed. It can add several hundred to well over a thousand dollars a month depending on the category, and it materially extends a runway. The Area Agency on Aging and county veteran services can point you to help filing.

Worked example. A semi-private room at $7,700 a month, all-in at $8,200 with add-ons. Income of $2,450 in Social Security. Burn rate: $5,750 a month, not $8,200. On $180,000 of countable savings, that is about 31 months rather than the 22 months the gross bill implies. Nine months of difference changes what a family should be doing.

One caution: once Medicaid begins paying, nearly all of that income goes to the facility as a patient-pay contribution anyway. The income offset extends the private-pay runway; it does not survive the transition.

Step Three: What Actually Counts as “What You Have”

The savings figure in the runway calculation is not net worth. Several of the biggest items on a Baldwin County family’s balance sheet do not belong in it, and several smaller ones do.

Generally not counted for Medicaid purposes, and therefore not part of the spend-down runway: the primary residence while the applicant lives there or has documented intent to return, or with a spouse or dependent in residence; one vehicle; household goods and personal effects; a burial space including plot, vault and marker; and an irrevocable prepaid funeral arrangement within state limits. Families sell exempt assets to “spend down” constantly, and it is pure loss.

Counted: checking, savings, certificates of deposit, brokerage accounts, and — this one matters on the Gulf Coast — a second property. A rental condo in Gulf Shores or Orange Beach, a lot in Foley, a camp on the Fish River is generally a countable resource at its equity value, and coastal Baldwin County property values have risen sharply. Retirement accounts are countable depending on payout status; confirm treatment locally.

Life insurance follows a rule of its own: policies insuring one person are aggregated by total face amount, and only if that combined death benefit exceeds a small threshold does the cash surrender value become countable. Term insurance has no cash value and adds nothing. Our explainer on how life insurance is counted as a Medicaid asset covers each type.

Build the countable list from documents, not memory, and take it to an Alabama elder law attorney before selling anything.

Countable savings Months at $8,200 all-in (gross bill) Months at a $5,750 burn rate (after $2,450 income) What that range means
$50,000 about 6 about 9 File the application correctly, once; ask about retroactive coverage
$100,000 about 12 about 17 Inside the look-back; every asset decision needs attorney review
$180,000 about 22 about 31 Inside the look-back; do not gift or deed anything
$250,000 about 30 about 43 Real planning window; see an attorney now, not later
$400,000 about 49 about 70 Transfers can potentially age out of the 60-month window
Step Three: What Actually Counts as "What You Have"

The Runway Table, and What Each Range Means You Should Do

Run the number, find your row, and act accordingly. These bands are practical guidance, not legal advice.

Under 12 months. The priority is a correctly prepared Medicaid application, filed once, with complete documentation. A denial for missing paperwork costs months you do not have. Do not sell exempt assets. Do not make gifts. Get an attorney involved now specifically to get the application right, and ask about retroactive coverage for months already private-paid.

12 to 36 months. You are inside the 60-month look-back window, which means every asset decision from here will be examined. This is the range where families do the most self-inflicted damage — gifting, deeding property to children, surrendering policies without comparing alternatives. Every one of those decisions needs attorney review before it happens.

36 to 60 months. There is room for real planning, and the window is closing. Structures that require time — certain trusts, restructuring property ownership, funding an irrevocable funeral arrangement — become available, and they take months to execute properly. This is the highest-value moment to see an attorney and it is the moment families almost never do.

Over 60 months. Full planning is possible because transfers can age entirely out of the look-back window. If a parent is healthy and there is a family history suggesting long-term care is likely, this is the conversation to have while it is still theoretical.

Alabama’s Advantage and Alabama’s Constraint

The advantage is real and worth stating plainly: Alabama’s nursing facility costs sit among the lowest in the United States, and Baldwin County tracks near or modestly above the state median because the Eastern Shore is an affluent submarket. The same dollars buy substantially more months here than in a coastal Northeast or West Coast market. If a family is weighing whether to move a parent to be near an adult child in a high-cost state, that decision costs money measured in years of runway.

The constraint is bed supply. Alabama regulates the addition of nursing facility beds through the certificate of need process administered by the State Health Planning and Development Agency, and new beds are not freely built in response to demand. Meanwhile Baldwin County has grown faster than any other county in Alabama, driven heavily by retiree in-migration from the Midwest and Southeast. The arithmetic of that is uncomfortable: more people needing beds, a supply that expands slowly.

Concretely, the county’s skilled nursing capacity is spread across a modest number of facilities — for example, the Robertsdale Rehabilitation and Healthcare Center is licensed for roughly 150 residents, and Bay Minette hosts a smaller center for living offering long-term and memory care. Hospital anchors are Thomas Hospital in Fairhope and South Baldwin Regional Medical Center in Foley. Verify current licensed capacity, ownership, staffing and inspection ratings yourself: federal inspection data for every certified facility is published on CMS Care Compare, and the Alabama Department of Public Health maintains offices in Robertsdale on Gilbert Drive and in Bay Minette on Courthouse Square.

Practical instruction: get on more than one waiting list, and do it before a discharge conversation rather than during one. Ask each facility how many of its beds are Medicaid-certified and whether it retains residents who convert from private pay — a long runway is worthless if the facility will not keep your parent at the end of it.

Alabama Medicaid in One Section

What happens when the runway ends. Nursing home coverage comes through the Alabama Medicaid Agency‘s institutional program, with the Elderly and Disabled Waiver as the home and community-based alternative. Applications for nursing facility Medicaid go to the Alabama Medicaid Agency through the district office serving Baldwin County — confirm which office and its current address before filing. The Elderly and Disabled Waiver for the Mobile, Baldwin and Escambia county area is administered through the Area Agency on Aging of South Alabama at the South Alabama Regional Planning Commission, and the Baldwin County Council on Aging runs county senior programs.

The rules, briefly: a $2,000 individual countable-asset limit as of 2026 (verify with the agency), an income cap for institutional coverage where income above the limit generally requires an income trust funded monthly, a 60-month look-back on transfers made for less than fair market value, and estate recovery after death. Our Baldwin County spend-down page covers the eligibility side in local detail, and our overview of nursing home spend-down covers the general mechanics.

Free, independent help: Alabama SHIP, the state’s federally funded health insurance counseling program delivered through the Alabama Department of Senior Services and local aging agencies, provides free Medicare counseling and sells nothing. For carrier and producer complaints, the regulator is the Alabama Department of Insurance. For legal and eligibility questions, use an Alabama elder law attorney; nothing on this page determines eligibility.

Where a Life Policy Fits in the Runway — and Where It Does Not

An in-force policy can extend a runway, and families forget they own one. Start by identifying what it actually is: pull the policy cover page and the most recent annual statement, and request an in-force illustration from the carrier for anything permanent. Baldwin County’s heavy Midwest in-migration means many households hold policies issued decades ago by carriers that have since been acquired or renamed — if you cannot identify the carrier, the Alabama Department of Insurance can help a consumer search for coverage.

Four honest paths, only one of which is a sale. First, check the rider schedule for an accelerated death benefit: if the insured is terminally or chronically ill, a qualifying rider may pay a portion of the death benefit directly, with no third party and no fees. This is money the family already owns and it is routinely missed. Second, a reduced paid-up election can stop the premium while preserving a smaller death benefit. Third, surrender for cash value, which is the floor rather than the ceiling. Fourth, a secondary-market review, which in the right facts pays more than surrender. Our guide to what a policy is actually worth compares them.

Where it does not help, said plainly. Term insurance with no remaining conversion right has no cash value and no market value — it is not a funding source. Face amounts under roughly $100,000 rarely attract secondary-market interest, which rules out most burial and final expense policies. An insured in good health for their age will see weak pricing or none, because offers are driven by life expectancy. If a surviving spouse genuinely needs the death benefit — a widow keeping a Fairhope or Foley home on reduced income — the coverage may be worth more than the cash. And timing matters if Medicaid is close: a settlement runs 60 to 120 days and the proceeds arrive as a countable resource that has to be planned for, not discovered. For a straight answer on a specific policy before money is committed, send the policy cover page for a free, no-obligation review; if it has no market value, you will be told that.

Frequently Asked Questions

How much does a nursing home cost in Baldwin County?

As of 2026, roughly $7,000 to $8,500 monthly for a semi-private skilled nursing room and $8,000 to $9,500 for a private room, with assisted living roughly $3,800 to $5,500 countywide and higher on the Eastern Shore. Add $300 to $1,200 for surcharges and ancillaries. These are survey-based ranges — get written rates from each facility.

How long will $150,000 last?

At an all-in $8,200 per month it is about 18 months of gross cost. But subtract income first: with $2,450 monthly in Social Security the burn rate is $5,750, which stretches $150,000 to roughly 26 months. Either figure sits inside the 60-month look-back window, so every asset decision from here needs attorney review before it happens.

Is Alabama really cheaper than other states?

Yes, meaningfully. Alabama’s nursing facility costs rank among the lowest in the country, and Baldwin County runs near or slightly above the state median because the Eastern Shore is an affluent submarket. The same savings buy roughly twice as many months here as in the most expensive Northeast markets. That gap is worth considering before relocating a parent.

Are beds hard to find here?

Tighter than the price suggests. Alabama restricts new nursing facility beds through its certificate of need process, while Baldwin County has been the state’s fastest-growing county, driven heavily by retiree in-migration. Get on more than one waiting list before a discharge conversation, and check current capacity, staffing and inspection ratings on CMS Care Compare.

Does the beach condo count against Medicaid?

Generally yes. The primary residence is usually exempt, but a second property — a rental condo in Gulf Shores or Orange Beach, a lot in Foley — is typically a countable resource at its equity value, and coastal Baldwin County values have risen sharply. Do not deed it to a child; that creates a transfer penalty. See an Alabama elder law attorney.

What is VA Aid and Attendance and does it help?

It is an increased VA pension benefit for wartime veterans and surviving spouses who need help with daily activities. It can add several hundred to well over a thousand dollars a month, which directly extends a private-pay runway. Baldwin County has a substantial veteran population and this benefit is routinely unclaimed. County veteran services and the Area Agency on Aging can help you file.

Should we cash in Mom’s life insurance to pay for care?

Compare four options first. Check the rider schedule for an accelerated death benefit if she is terminally or chronically ill — that pays with no fees. Consider a reduced paid-up election to stop the premium, surrender for cash value, or a secondary-market review if the death benefit is roughly $100,000 or larger. Surrender is the floor, not the ceiling.
